Peripheral Nerve Stimulation for the Management of Chronic Pain Syndrome: A Case Study of Complex Regional Pain Syndrome Posttrauma and After Surgery in the Leg

Our case study highlights the efficacy of PNS in the management of chronic pain syndromes, specifically CRPS, following major trauma and surgical intervention. Our case report involves a patient experiencing debilitating postoperative pain who underwent PNS treatment targeting the right infrapatellar branch of the saphenous nerve and posterior tibial nerve. Remarkably, the patient demonstrated a significant improvement in subjective pain levels with a reduction of approximately 75%, as well as enhanced functional capacity, compared to before the intervention.
Consensus Guidelines for the Use of Peripheral Nerve Stimulation in the Treatment of Chronic Pain and Neurological Diseases: A Neuron Project from the American Society of Pain and Neuroscience

Peripheral nerve stimulation (PNS) has evolved substantially over recent decades in terms of hardware and evidence supporting efficacy. Treatment targets continue to expand and address both pain and functional applications. The American Society of Pain and Neuroscience (ASPN) seeks to substantially update and expand upon a review of the evidence supporting PNS as well as provide guidelines for clinical practice. A diverse multidisciplinary panel of experts was selected to provide opinions and guidance based on evidence-graded assessment and clinical knowledge. This document aims to serve as a resource for clinicians and payors in the interest of expanding awareness of the breadth of research in the field of PNS and expanding access to therapy.
General Innovations in Pain Management

Chronic pain management is constantly evolving, and our literature review aims to describe the general innovations happening within the field. The need for advancements in chronic pain is a necessity, as debilitating back pain and other forms of chronic pain are significant issues in the United States. Traditionally, medications have been the initial treatment options in cases of chronic pain; however, the advancement in pharmacogenetics has led to an increased ability to create more personalized medication plans. Additionally, neuromodulation in spinal cord stimulation, transcranial magnetic stimulation, transcranial direct-current stimulation, and dorsal root ganglion stimulation continue to see increased usage in mainstream chronic pain management. These techniques have continued to prove successful in many chronic pain management cases. They are allowing practicing physicians more confidence in the variety of treatment options. Lastly, great strides have also been made in stem cell and regenerative therapies, such as platelet-rich plasma injections, and artificial intelligence, further advancing the various treatment options and overall efficiency of pain management. This review aims to critically analyze and review the most up-to-date literature within each section mentioned and comprehensively discuss the future of innovation in chronic pain management.
Successful Pain Management of Uncontrollable Left Wrist Pain With Peripheral Nerve Stimulation Following Failed Surgeries: A Case Report

This case study examines a 50-year-old patient with a history of CRPS type II of the left superficial radial nerve, who underwent 15 interventions, including 6 unsuccessful surgeries since 2021, and experienced extreme, uncontrolled pain after a work-related injury. After the permanent implantation of peripheral nerve stimulator (PNS) in November 2022, the patient enjoyed remarkable pain relief for the past one and a half years, with no signs of complications or necessity for further interventions. The PNS trial, permitted by work compensation rules, utilized a single incision technique, anchoring the leads to the brachioradialis fascia using a figure-of-eight 2.0 silk suture configuration.
Clinical Outcomes for Minimally Invasive Sacroiliac Joint Fusion with Allograft Using a Posterior Approach

Sacroiliac joint (SIJ) dysfunction can occur as a result of injury, degeneration, or inflammation. This dysfunction presents symptoms of pain at various locations, including the low back, hips, buttocks, and legs. The diagnosis of SIJ dysfunction is challenging and cannot be achieved solely with imaging studies such as X-rays, MRI, or CT. The current gold standard diagnostic modality is intra-articular SIJ blocks using two differing local anesthetics. Current treatments for SIJ dysfunction may be beneficial for short-term relief but lack long-term efficacy. The purpose of our study was to examine the outcomes of patients who underwent minimally invasive, posterior SIJ fusion using allograft at a single center.
